The Mustang North Middle School coaching staff would like to invite you to be a part of the Bronco Athletic Program during the 2024-2025 school year. The following information is important if you plan to play a fall sport.

Each student must have a completed OSSAA Physical Form turned in before he/she can participate in practices and/or games. Mustang’s Physical Day will be held May 10th. Physical Day athletes will need: OSSAA Physical Form, Parent Authorization for Student Travel, and $20. We encourage all athletes to participate in Physical Day. You can always go to your physician for your physical as long as you have your doctor complete the OSSAA Physical Form.

Along with your completed physical, students and parents must electronically complete RankOne Forms at www.mustangbroncos.org before the athlete can participate. Please complete your athletes RankOne Forms beginning July 1st.

Concussion Acknowledgement Form
Insurance Consent and Travel Form
OSSAA Eligibility Form
Sudden Cardiac Arrest Acknowledgement Form
NCAA Amateurism
Parent Communication Guidelines
Biological Sex Affidavit Form

Other competitive sports that students can participate in at MNMS include wrestling, basketball, track, tennis, and golf. If you plan to participate in any of these sports you are highly encouraged to participate in Physical Day or have your physician complete your OSSAA Physical Form this summer as well as completing your RankOne Forms prior to the season starting.
We hope that you will make a serious consideration to become part of the Bronco Athletic Program at Mustang North Middle School for this upcoming year.
